Release of Information

There are times when you may want to give us permission to discuss your information with someone else – like your parent or spouse.  You can complete a Release of Information Form and let us know who can access your information. Without a release, we will only discuss your information with you.
 
This form should be completed and turned into the Student Connection on the second floor of the Student Center building.
